An Emergency Response Checklist

Hey parents! It's up to you to make sure your kids can say "yes" to each and every one of the following points...and remember it's never too early to start reinforcing some of these important safety tips.

  1. We have emergency numbers posted near every telephone in our home. YES!
  2. We have memorized our home address and telephone number and they are also posted on every telephone in our home. YES!
  3. We know the telephone number of an adult to call in case of an emergency. YES!
  4. We learned how and when to dial 9-1-1 or our local emergency numbers. YES!
  5. We know to stay calm, speak clearly and stay on the telephone until the other person hangs up if we have to call for help in an emergency. YES!
  6. We have taken a certified First Aid or CPR course. YES!
  7. We have a complete first aid kit in our home. YES!
  8. Our family's special medical needs and health insurance information are handy. YES!
  9. In case of an emergency, we have two escape routes from our home and an outside meeting place. YES!
  10. Our house number is clearly visible from the street. YES!

The Authorization to Treat Minors should be left with your child's caregiver. This form allows physicians and dentists to treat your child immediately should an emergency occur. The form is valid throughout Oklahoma.
