Preparing Your Child for Surgery
To better prepare your child for surgery at The Children’s Hospital at Saint Francis, please complete the secured online pediatric anesthesia questionnaire by clicking the link below. Upon completion, you will be notified if a pre-operative visit is required.
Complete the online pediatric anesthesia questionnaire at least three working days before the scheduled surgery date, excluding weekends and holidays. If you are unable to complete the online questionnaire, please call (918) 494-7430, option #1 to schedule a pre-operative appointment.
